In 1995, in Baytown, Texas, a dedicated, community-minded Registered Nurse –determined to make a quality difference in home health care—started her own health care agency. Her focus, commitment, and passion for nursing, patient care, and advocacy for patients and staff, are only a few of the many attributes that have helped to elevate Signature Health Services to become the premier home healthcare agency in southeast Texas.
Nearly thirty years later, that same dedicated nurse is our President, and Signature Health Services has garnered Medicare’s coveted 4.5-star rating for patient satisfaction. Signature delivers the highest quality home care to thousands of patients in seven counties, including Brazoria, Chambers, Ft. Bend, Galveston, Harris, Liberty, and Montgomery County. Signature boasts outstanding team members with longevity, who have advanced in their careers through education and growth at work. To give one example: in 2007, one team member started her career with Signature Health Services as a field LVN, giving excellent hands-on care to our patients. As her passion began to grow for home health, she furthered her education while still working, obtaining her RN in 2011. She transitioned into being one of our top producing RN field nurses specializing in wound care and trained under one of our Medical Directors who is a Certified Wound Specialist through the American Board of Wound Healing. She has advanced to become our Director of Nursing. That’s how we motivate our best to embrace growth, excellence, and teamwork.
Signature Health Services carefully screens, vets, and monitors our nurses to ensure that only the top-tier of skilled, caring nurses are qualified to represent us and deliver the excellence that we demand, our referring physicians deserve, and our patients expect.
A deep passion for serving home-bound patients with compassion, and a commitment to building excellence, integrity, and teamwork inform and propel Signature’s noble mission. Over the years, Signature Health Services has worked with nearly 4,000 physicians to deliver the highest quality home health treatment and assistance to thousands of patients who received dignified and expert home healthcare.
All aspects of patient care are handled internally and onsite. That encompasses our outstanding scheduling department, therapy coordinator, intake, billing, orders, clerical coordinator, and director of nursing. Signature Health Services hosts a team of outstanding, accomplished, and tenured Supervising Registered Nurses who are in-house and ready to assist patients, caregivers, physicians or their staff, including through our after-hours, 24/7 on-call service. Signature operates under a case management model to provide continuity of care for our patients. What this means is that one of our in-house supervising nurses is responsible for a group of patients, in addition to that same group of patients being assigned to a primary field RN and LVN to ensure closely-monitored and excellent healthcare.
